Bilingual Community Support Team Therapist

** This is a NEW position to support a NEW Community Support Team for adolescent clients in the Behavioral Health Line of Service. This position will be community based with a home office at 6918 Windsor Avenue, Berwyn, IL. **

SUMMARY

Provides therapy, outpatient clinical assessments, and  provision of direct services to adolescent clients in the Community Support Team (“CST”) program through mental health rehabilitative services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides individual, family, and group therapy to clients receiving CST services.
  • Gathers member information to complete clinical assessments (IM+CANS) and formulate an accurate diagnosis. With the client and CST team members utilizes a strengths-based perspective and formulates a treatment plan based on their assessed need.
  • Determines frequency, duration, and intensity of treatment, identifies and investigates existing natural resources and supports, explains options available to CST clients and assists in the application process.
  • Provides direct service to CST clients through mental health rehabilitation services and supports, including illness self-management, skill building, identification and use of adaptive and compensatory skills, identification and use of natural supports and use of community resources. 60% of services must be provided in the member’s natural setting.
  • Completes clinical documentation in accordance with the Behavioral Health and organizational policies and procedures, established timeframes, and while meeting established productivity standards.
  • Participates in evaluation of referrals and facilitates placement into CST program with other team members, including ensuring potential clients meet eligibility requirements and all required screenings/assessments are completed; coordinates termination of services, ensures termination criteria are met, and ensures appropriate discharge plan.
  • Through supportive counseling, psycho-education and case management, assist people in the program to achieve their treatment goals, maximize independence and access needed community resources.
  • Coordinate with other community agencies and organizations such as DCFS, schools, hospitals, other providers in the community.
  • Provide services to facilitate an effective transition in living arrangement, consistent with the client’s welfare and development (e.g., a client being discharged from inpatient psychiatric care, loss of housing, etc.)
  • Provide intensive outreach in an attempt to engage clients in services and those who do not follow through with post discharge/deflection appointments.
  • Participates in specific program meetings, including the mandatory weekly staff meeting and required service line and agency trainings, meetings and consultations.
  • Other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE

Master’s degree in Counseling, Psychology, Social Work, Marriage and Family Therapy or a related field required.

Experience working with adolescent clients is required.

Experience conducting therapy inclusive of providing clinical assessments, treatment planning, and diagnoses is required. Substance abuse treatment experience preferred.  Experience completing progress notes and clinical documentation is required.  Experience using the IM+CANS is preferred.  Training in and experience with a variety of evidence-based treatment models required.  Ability to work as part of a team and collaborate with consumers, referral sources, and other community providers is essential.

LANGUAGE SKILLS

Bilingual (English/Spanish) required.

WORK ENVIRONMENT  

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.  Work environment may include client homes and a general office environment.  Local travel, including transporting clients, is required.

OTHER

Must maintain a safe driving record and have current valid Illinois Driver’s License.  Personal vehicle needed.  CSS staff will transport clients and their belongings in his/her personal vehicle.  Mileage for authorized work-related purposes is reimbursable.
