Bilingual Community Support Team Leader

** This is a NEW position to support a NEW Community Support Team for adolescent clients in the Behavioral Health Line of Service. This position will be community based with a home office at 6918 Windsor Avenue, Berwyn, IL. **

SUMMARY:  In collaboration with the Director and Program Coordinator, assist and support the management of the daily operations of the Community Support Team (CST) to ensure the effective delivery of services, efficiently utilize program resources, provide training and professional development of staff, and ensure the program is meeting community needs and funding requirements.  Organize and assist in all aspects of the CST services provided by Pillars Community Health.

Responsibilities include provision of direct services to adolescent clients in the CST program through mental health rehabilitative services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include hiring and training staff; planning, assigning, and directing work; evaluating performance, providing coaching and development (inclusive of providing clinical support, clinical, and administrative supervision for direct service work) to enhance professional development and ensure quality services, addressing concerns and resolving problems, and ensures employees complete required documentation and training requirements.
  • As a Team Leader, supervises a team as they provide mental health services in the community (60% of services must be provided in the client’s natural setting). This includes but is not limited to sharing case management duties within the team, leading by example while working directly with adolescent clients, providing guidance, and growing the clinical skills of the Community Support Team.
  • Oversees all service delivery and ensures that clinical services are delivered as indicated by program requirements, such as Rule 132 and other applicable regulations.
  • As a clinician on the team, provides individual, family, or group therapy to adolescent clients receiving CST services. Also provides direct service through mental health rehabilitation services and supports, including illness self-management, skill building, identification and use of adaptive and compensatory skills, identification and use of natural supports and use of community resources.
  • With the client and CST team members and utilizing a strengths-based perspective, assists in formulating treatment plans based on assessed need; reviews treatment plans according to policy; if meets applicable licensure requirements, complete LPHA reviews of staff IMCANS assessments as needed
  • Assists CST team members in completion of required Rule 132 paperwork and acts as the point person for staff questions and client consultations as they come up in the field.
  • Functions as the convener of the CST (Community Support Team) meeting, takes notes/minutes of CST weekly meetings,
  • Assists the Coordinator with making recommendations for treatment, identifying and investigating existing natural resources and supports, explaining options available to CST clients and assisting in the application process.
  • Completes all required paperwork (IM-CANS, treatment plans, progress notes, etc.) within required timeframes and per Rule 132 and agency policy. Ensures individual and team’s documentation meets regulatory, billing, and clinical standards.
  • Participates in evaluation of referrals and facilitates placement into CST program with other team members, including ensuring potential clients meet eligibility requirements and all required screenings/assessments are completed; coordinates termination of services, ensures termination criteria are met, and ensures appropriate discharge plan.
  • Through supportive counseling, psycho-education and case management, assists clients in the program to achieve their treatment goals, maximize independence and access needed community resources.
  • Coordinate with other community agencies and organizations such as DCFS, schools, hospitals, other providers in the community.
  • Provide services to facilitate an effective transition in living arrangement, consistent with the client’s welfare and development (e.g., a client being discharged from inpatient psychiatric care, loss of housing, etc.)
  • Provide intensive outreach in an attempt to engage clients in services and those who do not follow through with post discharge/deflection appointments.
  • Participates in specific program meetings, including the mandatory weekly staff meeting and required service line and agency trainings, meetings and consultations.
  • Other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE

Master‘s Degree in counseling, psychology, clinical social work or related field required.

At least (2) two years experience working in mental health services is required, with at least (1) one of those years working with clients living with serious mental illness.

At least two (2) years working with adolescents in a clinical setting.   Experience working with Rule 132 and Rule 115 in a mental health setting preferred. Substance abuse treatment experience preferred.

LCSW/LCPC licensure preferred.  Supervisory and program coordination experience preferred.

LANGUAGE SKILLS

Bilingual (English/Spanish) required.

WORK ENVIRONMENT  

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.  Work environment may include client homes and a general office environment.  Local travel, including transporting clients, is required.

OTHER

Must maintain a safe driving record and have current valid Illinois Driver’s License.  Personal vehicle needed.  CST staff will transport clients and their belongings in his/her personal vehicle.  Mileage for authorized work-related purposes is reimbursable.
