School Health Case Manager

** This is a NEW position in partnership with Anne M Jeans Elementary School in Willowbrook, IL.  It will work between AMJ and the Health Center in La Grange, IL. **

Brief Description

Provides expertise and support for the coordination of school health services. Works with students who are at risk of failing academically due to health-related issues, such as chronic health conditions or emotional concerns that interfere with their ability to participate regularly and fully in school activities. Case Manager works collaboratively with the student, the district’s School Nurse, staff, administrators, providers, parents/families, and community agencies to address factors that contribute to student absenteeism, poor academic performance, and limit student potential.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Collaborative Partner Coordination of Services:

  • Assists the Anne M. Jeans elementary school nurse in maintaining compliance with school physical requirements.
  • Follows-up on outstanding dental and medical records to ensure compliance with district requirements for attending school.
  • Obtains consent for services from parent/guardian.
  • Promotes clear communication amongst care team, clinicians, parents, faculty, and other staff by providing awareness of available services, hours, and helping to coordinate care/services to meet student and family needs.
  • Coordinates continuity of patient care with external payers.
  • Helps develop and maintain individualized emergency care/action plan to help address students with asthma, food allergies, diabetes, seizures, and/or other health issues like mental health or behavior disorders.
  • Assists with screening for social determinants of health that may impact student health and school attendance.
  • Utilizes electronic health record (eCW) to document patient contacts, related patient education, resource connection, and external care providers.

Patient Centered Care:

  • Manages potential risks and liabilities within the clinic to minimize risk of errors, accidents, and other adverse incidents by completing incident reports in a timely manner and submitting to supervisor.
  • Monitors EMR system notes and alerts, routing information appropriately to correct team members for quality patient care.
  • Supports patient/parent/guardian with the self-management of their diagnosis and behavior modification interventions.
  • Communicates with patients, patient support systems and peers from a trauma informed perspective.
  • Conducts patient nursing assessments, point of care testing, disease management, and patient education for chronic conditions and/or preventive care needs of individuals served at the clinic.

Other:

  • Attends all meetings as required.
  • Adheres to all organization policies and procedures.
  • Adheres to OSHA guidelines for a safe work environment.
  • Follows all rules required to comply with confidentiality and HIPAA policies.
  • Complies with all applicable laws, including but not limited to medical records laws, HIPAA, OSHA and the corporate compliance program.
  • Ensures compliance with organization health & immunization/vaccination requirements.
  • Performs other duties as assigned.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE

  • Associates or Bachelor’s degree in nursing nursing, field required.
  • IL LPN or RN License required. At least 1 year of clinical nursing experience required. Must have experience administering injections. Experience in a not-for-profit organization preferred. Experience in a hospital or community health center preferred. CPR Certification required
  • Case management experience preferred.
  • FQHC background or experience in school-based health center setting highly preferred.

LANGUAGE

  • English proficiency (read, write, speak) required.
  • Bilingual Spanish/English skills preferred, but not required.

SCHEDULE

Schedule will reflect school hours, with flexibility for providing hours before and after school hours, and occasional evenings.

OTHER

Must maintain a safe driving record and have current valid Illinois Driver’s License.  Personal vehicle needed for travel between PCH sites and Anne M Jeans elementary school.  Mileage for authorized work-related purposes is reimbursable.
