Click here to access the Patient Health Questionnaire (PHQ-9), referenced in the video. This is one of many tools we use to assess whether a medical patient is experiencing depression. If you or a loved one scored in the high range, this might be an indication of a serious mental health need—call our Crisis Team at 708-PILLARS (708-745-5277), select 1 for English/2 for Spanish, then 3 for Mental Health.  If you or a loved one score in the medium range, this may be an indication it’s time to seek mental health services.

At Pillars Community Health, we provide integrated primary medical and behavioral health care, a model developed by the Substance Abuse and Mental Health Services Administration (SAMHSA). Staff take a team approach to treating the whole patient—not just their physical needs but also their mental health and social needs. By regularly meeting, sharing information, and having access to patient records, staff from our Medical, Dental, and Behavioral Health teams can coordinate care to improve the overall health of each patient.
