This event is presented in partnership with the Summit Community Network, a coalition of local organizations working together to expand access to resources in Summit. With the support of a community grant, PCH and its partners are offering this health fair to address pressing community needs, including helping residents impacted by the loss of Medicaid coverage.
PCH staff from across departments will be on-site offering free activities and resources, including:
- Medical Services: Blood pressure screenings and education on diabetes, prediabetes, and blood sugar management.
- Benefits & Enrollment: Information on PCH’s discount program, Medicaid redetermination, marketplace enrollment, and assistance with documentation and follow-up appointments.
- Youth & Wellness: Interactive activities with The Loft at Eight Corners, including mindfulness exercises, art-based projects, and a prize wheel with giveaways.
- Substance Use Disorder Support: Details on expanded walk-in hours, service locations, and recovery resources.
Community partners will also be on site with resource tables, including NAMI Metro Suburban, Aging Care Connections, The Loft at Eight Corners, Morton College’s Project CARE, Age Options, BEDS Plus, Arab American Family Services and others.
