New Year, New Goals: Achieving Wellness in 2025

The start of a new year is a great time to set goals for yourself especially if those goals are around your physical, oral, and mental health. At Pillars Community Health (PCH), we believe your goals should be achievable and realistic, helping you build the life you want step by step.

When it comes to setting goals, focus on whole-person care, which includes your physical, mental, and social needs.  At PCH, we’re here to help with services built to meet your unique needs and circumstances. A great way to begin is by visiting a PCH location for an annual physical exam or behavioral health screening. We work to make care affordable and accessible for everyone, offering sliding-scale fees and welcoming uninsured patients by guiding you through the enrollment process.

Your Physical Health

Goals regarding your physical health can range from small changes like adding nutritious foods to your meals or finding free ways to stay active, such as walking or stretching at home, to more complicated ones, like reducing your alcohol consumption. If your goals include addressing substance use, PCH can provide you with substance use screenings and services.

For chronic conditions, such as help with the management of Diabetes, Hypertension, Asthma, Obesity, and high Cholesterol, PCH offers compassionate, comprehensive care, tools and resources. When it comes to your physical health, don’t forget about your dental needs. If your goals are to take better care of your teeth, PCH offers accessible dental services including exams, cleanings, dental sealants, fluoride treatment, and digital X-rays, so that you can feel confident about your smile and overall well-being.

Your Mental Health

Mental health is just as important as physical health. Reducing stress can include free or low-cost activities like yoga with a YouTube video, meditation on a park bench, or simply taking deep breaths to center yourself. PCH offers a range of behavioral health screenings including depression and anxiety, as well as services like brief intervention therapy, outpatient therapy, and coordinated psychiatry services.

Your Social Needs

Social goals look different for everyone. You might want to spend more time connecting with loved ones or focus on carving out moments for yourself. Both are important. Even if your schedule is packed, self-care can be as simple as taking a few minutes to do something that brings you joy or sets you up for the future.

PCH is also here to help if your social goals involve finding resources or support. Whether you’re working toward housing stability, food security, or transportation solutions, we can connect you with community resources and our partners to ease your journey.

Staying on Track

When setting realistic goals, be patient and proud of the progress you are making. Be patient with yourself and celebrate every small victory. Adjusting your goals or timelines is okay—it’s all part of the journey. Always remember that there are people supporting you in your journey, whether they are friends, family, or community. Pillars Community Health is here to guide you as you begin making your new year’s goals.
