HRSA, an agency of the U.S. Department of Health and Human Services, funds health centers to expand access to care in underserved communities. Every three years, HRSA conducts a detailed review of health centers’ governance, clinical services, and financial operations. Following our successful site review, PCH received recognition for Advancing Health Information Technology (HIT) for Quality, Diabetes Health, High-Value Care, and Health Center Quality Leader.
Advancing HIT for Quality
This recognition celebrates how PCH uses technology to improve care, including offering telehealth services, securely sharing information with other providers, engaging patients through digital tools, and collecting data to better understand community needs.
Diabetes Health
This badge highlights our success in meeting or exceeding two of the three diabetes measure targets.
High-Value Care
This recognition celebrates how PCH met or exceeded the Quality Score: 18.0 (Top 20% of 2023 and 2022 UDS) AND met or fell below the Percentage Cost Growth Benchmark: 2.54% (Top 30% of 2023 and 2022 UDS).
Health Center Quality Leader
This badge highlights that PCH was in the top three tiers for best overall clinical quality measures (CQM) performance based on average Adjusted Quartile Rankings (AQR). PCH was in the top 21-30%, receiving bronze.
Improving Health Care Access
This badge highlights our success in expanding services and reaching more patients. PCH achieved measurable growth in total patients served, including those receiving mental health, substance use disorder, vision, dental, and support services.
These awards reflect the dedication of our staff, partners, and supporters who make whole-person care possible every day. Together, we are ensuring that more people in our communities can access the compassionate, comprehensive care they deserve.
Salud Mental
Our behavioral health services are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). We have received the highest level of accreditation at each survey since 1995. We recently received our updated accreditation through 2024. Our Substance Use Disorder Services are licensed by the Illinois Department of Human Services Division of Substance Use Prevention and Recovery.
