As part of this ongoing evolution, we are making thoughtful, strategic transitions at two service locations: our school-based site at Anne M. Jeans Elementary School in Willowbrook and our interim site at 105 Burlington Avenue in Riverside.
These changes are not reductions in service but rather intentional shifts designed to consolidate resources, improve coordination, and strengthen our delivery of care across every community we serve.
What’s Changing — and Why
- The Burke Family Health Center at Anne M. Jeans, our school-based health center, will sunset its operations this spring. We are incredibly proud of the support we’ve provided to students and families at this site over the years. While our on-site services at the school will conclude, our commitment to the Willowbrook area continues through ongoing partnerships, community outreach, and access to services at our other nearby locations.
- Services at 105 Burlington Avenue will be temporarily suspended as of May 1, 2024. Originally opened as a short-term solution, this location has served its purpose and allowed us to continue meeting community needs during a time of transition. We are now focusing on building a more permanent, fully integrated model of care that will provide a better experience for patients and staff alike.
What Remains Constant
While locations may shift, our commitment to care never wavers. These transitions allow us to:
- Enhance whole-person care through better coordination across service lines
- Invest in trauma-informed, thoughtfully designed spaces like our new site at 321 N. La Grange Rd.
- Ensure long-term sustainability by focusing on service delivery models that are efficient, accessible, and rooted in community voice
We know change can bring questions — and we’re here to answer them. Patients and community partners affected by these changes will receive direct communication and support to ensure continuity of care. We will continue to show up in the places that matter most — in schools, neighborhoods, clinics, shelters, and beyond — as a trusted partner in health and healing.
