Older Adults Therapist/LCSW (Community Based)

SUMMARY

Provides outpatient mental health assessment and treatment services to a caseload of clients in the office or in the community depending on the needs of the clients

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide individual, family, or group therapy to a caseload of clients as determined by program needs.
  • Complete assessment in order to formulate an accurate diagnosis and comprehensive treatment interventions to serve the identified client and family.
  • Assess and stabilize high risk individuals and/or families.
  • Work in collaboration with other agency programs and staff.
  • Coordinate with other community agencies and organizations such as DCFS, schools, hospitals, other providers in the community.
  • Provide services to facilitate an effective transition in living arrangement, consistent with the client’s welfare and development (e.g., a client being discharged from inpatient psychiatric care, transitioning to adult services, or a DCFS client moving from one placement to another or to his/her parent’s home).
  • Provide intensive outreach in an attempt to engage clients who do not follow through with post discharge/deflection appointments.
  • Provide case management services as defined in the service plan that coordinate, support, and advocate for clients who need multiple services and require assistance in gaining access to and in using mental health , social, vocational, educational, child welfare, and other community services and resources.
  • Provide psych education and medication training to client and/or family as required.
  • Respond to clients and/or family needs outside of regularly scheduled session times.
  • Maintain records in accordance with agency policies and complete required outcomes protocols.
  • Participate in agency meetings and in-services.
  • Track program statistics and outcomes related to services provided and as required by funding sources
  • Attend and participate in community meetings Maintain community contacts and collaborations
  • Screen older adults referred for services to ensure appropriateness of program placement (PEARLS, Home based or outpatient therapy)
  • Provide consultation to agency staff related to issues and treatment affecting older adults
  • Other duties as assigned.

QUALIFICATIONS

EDUCATION/EXPERIENCE

Master’s degree in Counseling, Psychology, Clinical Social Work, Marriage and Family Therapy or related degree required.

LCSW required.

Related experience should include work with older adults, a demonstrated flexibility in the use of treatment modalities including individual, family, and group treatment for short and long term lengths of stay. Ability to work as part of a team and collaborate with consumers, referral sources, and other community providers are essential.

LANGUAGE

Bilingual (English/Spanish) skills preferred.

OTHER

Must maintain a safe driving record and have current valid Illinois Driver’s License and vehicle. Personal vehicle needed for transportation to/from community and home based interventions. Mileage for authorized work-related purposes is reimbursable.
