Family Nurse Practitioner

* We are growing and expanding our services!  We are looking to hire a FT or PT Nurse Practitioner.  Schedule would include 3 days at a local school working 3:30-8 pm **

Working under the direct supervision of the Chief Medical Officer and also the collaborating physician, the Family Nurse Practitioner completes all the duties associated with a licensed, advanced practice nurse and to manage patient care in accordance to the professional guidelines of Pillars Community Health.

Works closely with other providers and clinical leadership team. Instructs, oversees and works alongside the RN’s and Medical Assistants in the delivery of care. Supports the policies, procedures and protocols and plans of the Federally Qualified Health Center (FQHC) as it relates to direct patient care, care management, development and implementation of clinic procedures and patient education. A quality assurance element is achieved in collaboration with the Quality Improvement Specialist. As a quality measure, assures accurate EMR management, and HIPAA and OSHA compliance.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Patient Care

  • Provides  general medical care and treatment to patients under the direction of a physician and as dictated by Illinois Registered Nurse License and Advanced Practice Nurse License and  agency policies and procedures
  • Provides primary patient health assessment
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physicians to diagnose and treat acute, chronic and long-term health issues of patients
  • Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs
  • Prescribes or recommends medication or other forms of  medical treatment such as therapeutic procedures

Case Management

  • May refer patients to physicians for consultation or to specialized health resources for treatment
  • Coordinates patient treatment, management and planning with the clinical staff and physician to provide continuity of care, including assistance with referrals to other agencies and health care providers
  • Submits health care plan and goals of individual patients for periodic review and evaluation by physician
  • Provides telephone support to patients, physicians hospitals, pharmacies and other health care providers

Patient Records

  • Responsible for  documentation of patient care; records physical findings, and formulates plan and prognosis, based on patient’s condition
  • Maintains patient information and confidentiality in accordance with applicable laws and Agency policies

Patient Education

  • Provides education to patients and/or their families about preventive care, medical issues and the use of prescribed medication and treatments
  • Promotes patient health through Agency programs

Clinical Quality Performance

  • Assists in the compilation of statistical data and patient outcomes for agency needs, as requested
  • Participates in agency quality performance programs
  • Helps to maintain safe and clean patient care environment
  • Embraces encounter requirements as established by HRSA (Health Resource & Service Administration) for FQHC’s (Federally Qualified Health Centers)
  • Adheres to OSHA guidelines for safe work environment

QUALIFICATIONS

  • Professional certification and degree from an accredited educational institution – ANA or ANCC
  • Illinois Advance Practice Nurse Licensure
  • At least 3 years of clinical advanced practice nursing experience.
  • Clinical experience with adult patients, children and well-woman care preferred
  • Experience in patient education, preference for education experience in areas of diabetes management, cardiac risk assessment and/or family planning
  • Current CPR Certifications
  • Current Professional Liability Insurance
  • Minimum of two years clinical experience in ambulatory medicine care and/or community health nursing experience.
  • Meets agency health & immunization/vaccination requirements

  • Analytical & Critical thinker
  • Computer proficiency in ECW, preferred; internet and email; Microsoft Office software
  • English proficiency required
  • Highly organized
  • Ability to multi-task and willingness to learn new tasks
  • Ability to perform basic mathematical computations using whole numbers, fractions, decimals
  • Ability to work within a team environment
  • Empathic and sensitive to patient needs; demonstrate strong case management skills for patient care
  • Resourceful & Assertive
  • Bi-lingual English/Spanish skills a plus
