Community Support Counselor

The Community Support Counselor is responsible for providing Individual, Family, or Group Therapy services and Community Support Services to a caseload of child, adolescent, and adults clients. Services are provided via telehealth, in the community, at a client’s residence, or in the office depending on the needs of the clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1) Provides individual, family, or group therapy and Community Support Services to a caseload of clients as determined by program needs.

2) Conducts initial home visits and phone calls with clients to explain the Community Support program and gain information about the clients and their needs.

3) Determines frequency, duration, and intensity of treatment, and develops treatment plans.

4) Completes clinical assessments to formulate an accurate diagnosis and comprehensive treatment interventions to serve the identified client and family.

5) Assesses and stabilizes high risk individuals and/or families. Provides case management services as defined in the service plan that coordinate, support, and advocate for clients who need multiple services and require assistance in gaining access to and in using mental health, social, vocational, educational, child welfare, and other community services and resources.

6) Provide transportation to and from activities as a part of the therapeutic plan.

7) Completes clinical documentation in accordance with the Behavioral Health and organizational policies and procedures, established timeframes, and while meeting established productivity standards.

8) Respond to clients needs outside of regular session times.

9) Participates in specific program meetings, including required staff, line of service, and organizational meetings, and required trainings, and consultations.

10) Other duties as assigned.

 

QUALIFICATIONS:

EDUCATION AND EXPERIENCE

• Masters degree in Counseling, Psychology, Clinical Social Work, Marriage and Family Therapy or related degree required.
• Experience working with children, adolescents and adults living with mental illness in an individualized setting is required.
• At least two (2) years experience working with children is needed for the CSI program work.
• Attention to detail and capacity to work independently is essential.

Experience conducting therapy inclusive of providing clinical assessments, treatment planning, and diagnoses is required. Experience completing progress notes and clinical documentation is required. Experience using the IM+CANS is preferred. Training in and experience with a variety of evidence-based treatment models required. Ability to work as part of a team and collaborate with consumers, referral sources, and other community providers is essential.

Bilingual preferred (English/Spanish)

OTHER

Must maintain a safe driving record and have current valid Illinois Driver’s License. Personal vehicle needed. Staff will transport clients and their belongings in their personal vehicle. Mileage for authorized work-related purposes is reimbursable.
