Quality Improvement Clinical Nurse Manager

SUMMARY

Establishes and manages the clinical support practices and protocols of the Health Center, including direct patient care, care management, development and implementation of clinic procedures, patient triage, patient education. Embraces and implements clinical goals and approach of the CMO and ensures medical clinic nursing staff, medical assisting staff, laboratory staff and care coordination service staff comply with expectations and best professional practices. Ongoing competency training and testing of nursing clinical staff is a key part of the position. As a Quality Improvement lead, oversees and manages the practices, the data, reporting and procedures of QI and Risk Management. Advises and leads the agency in these efforts in collaboration and under the direction of the CEO, CMO and the SVP of Health Center Services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Medical Services Delivery:

With CMO or as delegated by CMO:

  • Oversees the day-to-day medical and nursing care coordination operations of Medical Services, including laboratory services and some direct clinical care
  • Collaborates with provider team for effective continuum of patient care
  • Establishes and updates standards of care and assures implementation of health plan.
  • Accurately tracks and manages patient referral process, leading those team members responsible to keep the referral process running efficiently and closing them when complete so they do not back up.
  • With the CMO tracks the closed patient encounter process of the provider team. Assisting providers in the process as may be necessary.
  • Assures nursing and ancillary staff implements patient care based upon appropriate standards of care and within respective scopes of practice.
  • Assures appropriate care management for all patients, and for disease or population-specific care management initiatives.
  • With CMO, develops and oversees implementation of patient education goals and objectives

Quality Management Responsibilities:

  • Manages the operation of Continuous Quality Improvement (CQI) program, including the development of the Quality Assurance Plan and implementation of quality improvement goals and objectives, as directed by the SVP.
  • Directly responsible to collect clinical outcomes data and evaluate, placing it in the monthly CQI dashboard, reporting appropriately as set forth by the CMO and/or SVP of Health Center Services.
  • Under the direction of and with the SVP of the Health Center, CMO, Dental Director, and Chief Behavioral Health Officer helps ensure the delivery of health care meets or exceeds the needs and satisfaction of patients served, organizational and professional standards, clinical outcomes and related expectations, productivity standards, goals and expectation.
  • As a support to and with the CMO, Dental Director, and Chief Behavioral Health Officer ensures metrics for clinical outcomes are consistently reviewed and that the Health Center is provided clear direction on ongoing improvements in the quality of care.
  • Oversee development and monitoring of quality improvement action plans applying the PDSA (Plan, DO, Study, Act) model for improvement and closes plans when the stated purpose is accomplished.

Personnel Management:

  • Directly supervises the RN’s, Medical Assistants, Care Coordination team and Laboratory Services personnel.
  • Hires, supervises and evaluates direct reporting staff in accordance with agency policies and procedures
  • Develops and conducts testing of clinical competencies for RN and Medical Assisting personnel
  • Develops and implements training and professional development of clinical support team members
  • Coordinate nursing student affiliates and internships with schools of nursing, medical assisting, etc.
  • Maintain relationships with area school nurses and attends school nurse meetings as possible for collaboratively building a healthy community

QUALIFICATIONS

EDUCATION & EXPERIENCE

  • BSN degree from an accredited school of nursing
  • Illinois Licensure as a Registered Nurse and certification in good-standing
  • Minimum of five years clinical experience in ambulatory medicine care and/or community health nursing experience.
  • Previous experience in clinical quality, risk management and PCMH certification highly preferred
  • Knowledge of clinical reporting requirements
  • FQHC background or experience in outpatient setting highly preferred
  • Clinical administrative and supervisory experience required.

LANGUAGE SKILLS

English/Spanish fluency preferred

DEMONSTRATED COMPETENCIES

  • Minimum 3 years of experience in public health, hospital and or community health center
  • Minimum 2 years supervisory experience
  • Minimum 1-year experience with EHR system – ECW preferred
  • Highly organized with ability to multi-task and willingness to learn new tasks
  • Ability to perform basic mathematical computations using whole numbers, fractions, decimals
  • Excellent oral and written communication and interpersonal skills
  • Ability to read/interpret/write/present policies and reports with comparative data of a complex nature
  • Ability to train/teach/lead peers and others on QI related matters
