COVID Outreach Education Specialist

** This is a NEW temporary full-time position to provide outreach to our communities through May 2021. **


By acknowledging the health disparities, this role helps combat the disproportionate impact of the COVID-19 pandemic in suburban Cook County’s African American and Hispanic/Latinx communities. Our multi-disciplinary, collaborative network, community, public and institutional partners are working to ensure Black and Brown communities have access to COVID-19 testing, up to date information and are quickly connected to resources.

Under the direction of the Vice President of Community Engagement and in collaboration with Suburban Cook County Department of Public Health (COVID-19 Community Support Grant), the COVID Outreach Education Specialist will provide ongoing outreach and education regarding COVID-19 and increased access to services to enhance client/patient connectivity to available treatment resources.


  • Identifies opportunities and methods for promoting and educating the community about COVID testing.
  • Serves as a resource for community members for the purpose of providing information and listening to concerns.
  • In collaboration with other outreach and care coordination personnel and efforts, conducts outreach to community members and other organizations in the near-west Chicago suburbs regarding COVID-19 and other health services.
  • Advocates for the individual and helps reduce the barriers to access services.
  • Acts as the primary contact for community and governmental members who have questions or concerns.
  • Conducts case management services and community linkages as needed.
  • Cultivates collaborative relationships, focusing on agencies providing COVID-19 resources and identifying barriers and service gaps.
  • Maintains up-to-date knowledge of CDC, IDPH and Cook County Department of Public Health guidelines, data and community health-related topics through trainings, webinars, community meetings, etc.
  • Submits required documentation to supervisor within designated timeframes.

Bachelor’s Degree in Health Education, Health Promotion, Occupational Health, Disease Management, Behavioral health or other closely related healthcare focus. Equivalent combination of education and experience may be substituted for degree. At least six (6) months of outreach experience required.


Bilingual (English/Spanish) required.


  • Ability to lead small groups and facilitate group conversations communicating clearly and effectively
  • Ability to interact with all levels of the community and professionally represent the organization publicly
  • Culturally Competent with a commitment to reducing barriers to healthcare for marginalized and/or low-income individuals
  • Excellent oral and written communication and interpersonal skills
  • Familiarity with collaborative services and community programs.
  • Demonstrated strengths of personal integrity, self-confidence
  • Highly organized with ability to multi-task and willingness to learn new tasks
  • Ability to track and input data
  • Computer proficiency in electronic health records preferred; internet and e-mail, Microsoft Office software and/or Google Docs required
  • Ability to work within a team environment
  • Critical and analytical thinker with ability comprehend and retain information for on-the-spot use
  • Self-Starter, resourceful, and assertive


Must maintain a safe driving record and have current valid Illinois Driver’s License and vehicle. Personal vehicle needed for transportation to/from community and home-based interventions. Mileage for authorized work-related purposes is reimbursable.
