Community Support Team Coordinator

SUMMARY

Responsibilities include day to day oversight of assigned staff in the Community Support Team (“CST”) and the Community-Integrated Living Arrangements (“CILA”) program.  Provides administrative and clinical support and back-up coverage to the Community Based Services Director and Vice President.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides direct supervision of CST and CILA staff, including providing administrative and clinical support, supervision and consultation
  • Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include training and hiring staff; planning, assigning, and directing work; evaluating performance, providing coaching and development, addressing concerns and resolving problems, and managing employee timecards.
  • Trains new employees on all aspects of the agency, CST and CILA services and training requirements as outlined in Rule 132 and Rule 115, including procedures, documentation, scheduling, referral to other services, collaboration with other departments and staff, compliance with productivity expectations and quality assurance standards.
  • Functions as the convener of the CST (Community Support Team), oversees all service delivery and ensures that services are delivered as indicated by program requirements, such as Rule 132 and other applicable regulations.
  • Evaluates referrals and facilitates placement into CST and CILA program with other team members, including ensuring potential clients meet eligibility requirements and all required screenings/assessments are completed; coordinates termination of services, ensures termination criteria are met, and ensures appropriate discharge plan.
  • Provides consultation and collaboration with agency staff to identify needs, goals, and activities that will benefit the client and will lead to proper placement and level of care.
  • Collaborates with other leadership staff in assuring compliance with agency wide policy and procedure, quality assurance expectations and in ensuring compliance with productivity standards.
  • Makes recommendations for quality and/or process improvements as well as the organization’s development in order to more effectively and efficiently perform the functions of this position. Develops or updates policies and procedures related to program
  • Provides direct services to both CST and CILA clients, including supportive counseling, psychoeducation, case management and intensive outreach in an attempt to engage clients in services and those who do not follow through with post discharge/deflection appointments
  • Maintains client records as needed by completing progress notes, IATPs and other necessary documentation to maintain compliance with agency policies.
  • Ensures appropriate oversight and monitoring of CILA clients. Ensuring 7 days a week coverage by staff and/or on-site monitors including developing a monthly coverage schedule and obtaining and/or providing coverage when needed.
  • As needed, responds to emergencies 24 hours per day/7 days per week per established coverage plan. Ensures proper back up emergency coverage when not available.
  • Conduct outreach to other agencies to encourage collaboration and outside referrals to Community Based Programs
  • Participates in program management meetings. Collaborates with other Pillars Community Health departments and staff to maintain compliance
  • Attends weekly supervision sessions.
  • Conducts special projects as assigned

QUALIFICATIONS

EDUCATION AND EXPERIENCE

Master‘s Degree in counseling, psychology, clinical social work or related field required.  At least two year’s experience working in mental health services is required.  Experience working with Rule 132 and Rule 115 in a mental health setting preferred.

LCSW/LCPC licensure required. License eligibility will be considered if clinical license can be obtained within 3-6 months of starting in the position.   Supervisory and program coordination experience preferred.

OTHER

Local travel, including transporting clients, is required. Must maintain a safe driving record and have current valid Illinois Driver’s License. Personal vehicle needed. Mileage for authorized work-related purposes is reimbursable.
