Responsible for outreaching, engaging, screening, and assessing individuals for behavioral health and medical services, and provides care coordination.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Persistently engages individuals in need of behavioral health or medical services through outreach and referrals. Outreach includes but is not limited to frequent attempts and contact with individuals through home visits, street level engagement, relationship building activities, or hospital visits.
- Completes specific Health Risk Assessments (HRA), Health Risk Screenings (HRS), Transition of Care (TOC) assessments, and IM+CANS assessment to enroll individuals into programming.
- Plans, facilitates, evaluates and advocates for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality outcomes.
- Advocates and coordinates linkage or referrals to improve health, social, and environmental conditions.
- Facilitates access to, and the monitoring of, services identified in a person-centered care plan to manage symptoms for optimal health outcomes and promote wellness.
- Collaborates with other treatment providers to meet an individual’s comprehensive health needs.
- Develops and maintains in-depth knowledge of available support services.
- Conducts outreach for and facilitates health and wellness education.
- Assists individuals in understanding and obtaining health care benefits.
- Assists individuals with discharge planning from the hospital.
- Develops a transition of care plan as needed.
- Maintains records in accordance with agency policies and complete required outcomes protocols.
- Participates in all mandatory training sessions and team meetings.
- Maintains confidentiality.
- Transports individuals to appointments as needed.
- Works in collaboration with other agency programs and staff
- Other duties as assigned.

EDUCATION AND EXPERIENCE
Bachelor’s degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, pastoral counseling, family therapy, or related human services field or a bachelor’s degree in any field with two years of supervised clinical experience in a mental health setting. Customer service experience required. Intermediate computer skills required.
This position requires one to meet the requirements of a “Mental Health Professional” as per Rule 132 under IDHS. In lieu of the degree requirements above, one may also have:
- a practical nurse license under the Nurse Practice Act [225 ILCS 65];
- a certificate of psychiatric rehabilitation from a DHS-approved program plus a high school diploma or GED plus 2 years of experience in providing mental health services;
- a recovery support specialist certified from, and in good standing with, the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc.;
- a family partnership professional certificate from and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc.;
- an occupational therapy assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with at least one year of experience in a mental health setting;
- a high school diploma or GED and 5 years supervised clinical experience in mental health or human services.
- A supervised internship in a mental health setting counts toward the experience in providing mental health services.
Must be Bilingual (English/Spanish) and be able to communicate well verbally and in writing in both languages.
Seeking candidate with a positive, caring and compassionate attitude toward mental health and those with mental illness. Must also be able to problem solve and make quick decisions during intake calls. Flexibility and ability to think independently are also valuable traits.
Scheduling is based on needs of the department and includes some evening hours and may include Saturday hours, depending on need.
Must maintain a safe driving record and have current valid Illinois Driver’s License. Personal vehicle needed for transportation to/from home, school and community-based interventions. May also transport clients to appointments. Mileage for authorized work-related purposes is reimbursable
